You will need certain information about your father to determine his eligibility and what information to actually place on the monument. The document which will provide the best and most concise information is his DD-214. Once the necessary proof of service has been established and other service information determined, the staff at City Hall will initiate the procedures to have him added to the monument.
The National Archives is restricted in the information that may be released without authorization from the next of kin; however, the necessary and pertinent information should still be available.
There is a fee of $75 for the paver which is used to offset the cost of the Memorial. In order to maintain accuracy, the pavers will require proof of service, as well. Also, if any research must be done by staff then there may be a cost for that research.
The Uniformed Services Memorial is a monument to those veterans of our military and non-military uniformed services who have given their lives in the course of their duty.
One of the easiest ways is to visit the National Archives website, http://www.archives.gov/veterans/evetrecs . The website explains who may request the records and allows requests to be made electronically. The website also lists other methods to obtain records.

The Veteran’s pavers are for any veteran from of any of the uniformed services, alive or deceased from our City and may be purchased by the veteran, a family member, or a friend.
